If you are a resident of Texas and wish to change your name legally, you will be asked to fill up a form related to name change. They charge $15 for any correction made on the birth or death certificates and then $22 for a new copy of the certificate, once the correction has been done.

If you need to change your child’s last name or even your last name and use the biological father’s last name on the certificate, this is an easy thing to do. You will not be asked for a court order for the name change. But if you are changing the name to something other than the biological father’s name, then you will need court approval before getting that done.

The court order forms for a name change legally can be found online or by visiting the court offices.

If you are looking to change the name on a death certificate, it is necessary that you fill up a VS271 application and that should have the signature of a licensed funeral director. You will also have to get a VS271.1, which is yet another consent form and this will have to be signed three people, the cemetery sexton, where the body is placed, the plot owner and the deceased person’s next of kin. This will cost you $25.

Changing the name is very easily done without any approvals whatsoever if the name is being changed to the biological father’s name, otherwise the name change, if being done to some other name, will need the above forms to be completed and duly signed and it also involves some expense and it may also take a little time before you get all the required signatures from the various people involved.
